Duplicate Death Certificate

The application must contain ALL the following:

  • Fill in the supplied form in capital letters.
  • Copy of the applicant’s passport.
  • Copy of previous Death Certificate.
  • Letter of request addressed to the Ambassador.
  • Non-refundable fee of €15.00

Note: Duration for issuance of Duplicate of Death Certificate is six to eight weeks.
